Inflammation is one of the crucial processes of innate immunity that alerts the immune system and recruits the needed cells, clears the damaged area, and then sets the stage for subsequent tissue repair or regeneration. Despite the importance of inflammation to immunity and the role of chronic inflammation in many common pathologies, we have only begun to gain a basic understanding of the major events that initiate, regulate, and inhibit the process.
Although we have known that neutrophils are among the first cells to arrive on the scene of an inflammatory response, we have only recently begun to understand the important role that these cells play in the process. It was discovered in the mid-2000s that neutrophils form structures dubbed neutrophil extracellular traps, or NETs, when activated in an inflammatory response. Briefly, what are NETs, what is in them, and what purpose do they serve in inflammation and immunity? What are a few of the major pathologies that NETs have been implicated in?

Scenario
Over the last two weeks, you have spent your time analyzing the data and information that you gathered during your literature review. Your supervisor in the health policy think tank is pleased with your literature review and has asked you to move forward with your analysis, considering various perspectives.
The think tank serves multiple health care organizations and policy makers. Health care organizational leadership will depend on your analysis to help them determine strategic, legally compliant measures and lobby policy makers for needed change. You must develop a solid factual analysis to support policy makers in evidence-based policy making.
Consider topics such as painful versus painless cost controls and reimbursement models or quality initiatives that could be improved as indicated by your analysis.

The most common cause of mortality in individuals with A1AT deficiency is emphysema, with liver disease following closely behind.
The disorder is due to defective alleles of the SERPINA1 gene, which encodes the A1AT protein. The protein’s normal physiological function is to act as a check on neutrophil elastase, an enzyme secreted by neutrophils in response to lung inflammation. The normal allele is denoted M. There are two mutant disease-causing alleles that are autosomal codominant: S and Z. Heterozygotes (MS and MZ) have a somewhat elevated risk of COPD, especially if they smoke; homozygous ZZ individuals have the most severe phenotype, with an extremely high risk of developing emphysema.
In many individuals with severe cases of A1AT deficiency (the ZZ genotype), intracellular accumulations in the liver are usually noted upon biopsy. What might be the cause of this phenomenon?
